Developing Individual Plan of Care and Providing Skilled Healthcare
- Communicate and collaborate with Progressive clinicians, other healthcare providers (primary physicians, hospitals, etc.) and family members/friends to develop a personalized plan of care.
- Provide skilled nursing services, treatments and diagnostic and preventative procedures.
- Work with Rehabilitation team to provide physical, occupational and speech therapy as needed.
- Engage social worker to assist with social and emotional challenges.
- Teach, supervise and counsel patient and family regarding nursing needs and follow-up care.
- Conduct ongoing communication with physicians.
- Discharge patient only after every individual goal from plan of care has been met.
