Each new patient referred undergoes a rigorous head-to-toe evaluation to assess his/her current physical, emotional and psychological condition. Then the case manager takes the following steps to develop a thorough plan of care and provide skilled healthcare.

Developing Individual Plan of Care and Providing Skilled Healthcare

• Communicate and collaborate with Progressive clinicians, other healthcare providers (primary physicians, hospitals, etc.) and family members/friends to develop a personalized plan of care.
• Provide skilled nursing services, treatments and diagnostic and preventative procedures.
• Work with Rehabilitation team to provide physical, occupational and speech therapy as needed.
• Engage social worker to assist with social and emotional challenges.
• Teach, supervise and counsel patient and family regarding nursing needs and follow-up care.
• Conduct ongoing communication with physicians.
• Discharge patient only after every individual goal from plan of care has been met.

In addition to thorough case management, Progressive’s skilled nurses deliver a high level of care and treatment unsurpassed in this industry. The team often collaborates with each other to disseminate knowledge about the latest techniques and practices. Progressive also provides and encourages specialized/continuing education and training for all its clinicians.

Wound Care Specialty Services

Progressive Home Care also has a strong Wound Care Program led by a highly experienced CWOCN to address wound care and healing. This program includes:

• Expert assessment and evaluation including digital photographic reporting of wound status, nutritional assessment, and portable dopplers for advanced vascular assessment
• Advanced treatment options such as Anodyne Therapy and WoundVacs for complicated wound management.
• Ostomy care to provide individualized ostomy care plans.
