An integral component of a patient’s care team, our professional Medical Social Workers support each patient’s in-home recovery by identifying and coordinating critical community resources. All of Progressive’s Medical Social Service staff have advanced degrees in Social Work (M.S.W.) and many years of experience serving patietnts in a medical setting.

From initial patient contact, the social worker evaluates the patient’s current living situation and identifies any environmental, psychosocial or financial challenges hindering the attainment of health care goals. Then the social worker helps the patient and family engage relevant resources to enhance the patient’s overall well-being.

Specific Services Provided:

• Evaluating social, economic, environmental and emotional factors that could affect the patient’s condition.
• Obtaining needed community services to assist with meals, chores, transportation, financial assistance and support groups.
• Counseling and continuing education for patients and families.
• Assisting with long-term planning and alternate living arrangements.
• Training for stress management and coping skills.
• Helping eligible patients obtain prescription and other assistance.
